You can play the game completely for free, there is no requirement to pay for anything. However purchasing a payed character has a few benefits that you may find useful if you are a frequent player.
Note: payed items are the same price for all characters, payed or free. All payed items can be used by free characters. Everyone is treated fairly when it comes to real money, no exceptions.
*protection from purges: every so often, the game will purge characters who have not been active for a long time, or who's name is too short (less than 5 characters long).
*unlimited item quantities: free characters can only have up to 100 of any given item, say for a few exceptions like water. This is more than enough to survive, but being able to have more supplies will let you prepare more and be lazy pretty much forever.
*giving large quantities of an item: free characters can only give a maximum quantity of 10 to other players at a time. So giving 50 of an item requires you to share 5 times.
*extra solo quests and quest lines: most quests are free, but this is a good selling point for those who want to experience all that stw has to offer.
*cheaper prices in the stw poinnts store: many items cost less to buy, so your quest rewards will last a lot longer since you save a lot of points.
*extra items in the stw points store: full list of additional items include: coins and survival packs.
*more gifts in the giftshop: When you have a free character, you can only pick up 10 gifts per day in the fishing village giftshop, but with a payed character you can pick up 20. The premium gift timer is also reduced from every 5 days to 3.
